A fistula or a fistula-in-and is an abnormal hollow tract or cavity that is lined with granulation tissue and that connects a primary opening inside the anal canal to a secondary opening in the perianal skin. Secondary tracts may be multiple and may extend from the same primary opening. An anal fistula is basically a tunnel that runs from inside the anus to somewhere in the skin around it. It usually follows an infection that didn’t heal the right way. Just inside your anus are several glands that make fluid. Sometimes, they get blocked or clogged. When that happens, a bacteria buildup can create a swollen pocket of infected tissue and liquid. This is also called an abscess. If you don’t treat the abscess, it’ll grow. Eventually, it’ll make its way to the outside and punch a hole in the skin somewhere near your anus so the gunk inside it can drain. The fistula is the tunnel that connects the gland to that opening. Most of the time, an abscess causes a fistula. Usually, a brief history followed by clinical evaluation – including a digital rectal examination is enough to diagnose an anal fistula. At the same time, Proctoscopy is also done to check for any concomitant condition in the rectum. An MRI Fistulogram may be needed for complicated fistulas in which the tract is difficult to delineate. Fistulography, an X-Ray of the fistula after injecting a contrast solution is not done any longer as it may lead to the formation of a false tract due to forceful injection of the dye.
